By entering Jesse’s Sports Bar & Grill or Jesse’s Liquor Store, I hereby acknowledge and agree to the following:

1. Voluntary Participation

I am voluntarily participating in all activities available at these establishments, including but not limited to:

• Drinking alcohol

• Eating food

• Playing games

• Using Delta & Nicotine products

I understand these activities involve inherent risks such as physical injury, property damage, and other potential hazards.

2. Assumption of Risk

I accept full responsibility for all known and unknown risks associated with these activities and any resulting injury, damage, or loss.

3. Release and Waiver

In consideration for being allowed to participate, I release and discharge Jackson 06 Inc DBA Jesse’s Sports Bar & Grill and Jesse’s Liquor Store, its officers, employees, agents, and affiliates from all claims or causes of action arising from my participation.

4. Indemnification

I agree to indemnify and hold harmless Jackson 06 Inc DBA Jesse’s Sports Bar & Grill and Jesse’s Liquor Store from all claims made by others due to my actions during my time at the establishments. I also agree to cover any legal fees or costs incurred as a result of my actions.

5. Damages & Fees

I agree to pay for any damages or legal fees incurred by Jackson 06 Inc as a result of my actions, including but not limited to:

• Legal expenses

• Increased insurance costs

• Repair or replacement of damaged property

• Lawsuit judgments (e.g., resulting from driving under the influence)

Use of Video and Likeness

1. Grant of Rights

I grant permission to Jackson 06 Inc DBA Jesse’s Sports Bar & Grill and Jesse’s Liquor Store to capture photographs, video, and audio of me during events and activities.

2. Usage Rights

I understand that my likeness may be used in promotional materials, social media, websites, and publications without compensation.

3. Waiver of Rights

I waive the right to inspect or approve the final media content and waive any rights to royalties or other compensation related to the use of my image or voice.

4. Perpetual Release

This grant is perpetual and binding upon me, my heirs, legal representatives, and assigns.

Acknowledgment

I have read, understood, and voluntarily agree to this Guest Participation Waiver & Media Release. I acknowledge that this is a legally binding contract between myself and Jackson 06 Inc DBA Jesse’s Sports Bar & Grill and Jesse’s Liquor Store, effective upon my entry into either establishment.
